Neferpitou is one of three Royal Guards born alongside the Ant King, made specifically to protect him, and strong enough that ordinary Nen users register as scenery. Pitou behaves like a cat throughout: crouching, playing, tilting the head at things that are about to die. The first appearance involves sensing Netero from an enormous distance and reacting with delight rather than fear, which tells you where the character sits.
The ability is Doctor Blythe, a Nen construct that performs surgery. It repairs bodies precisely, and Pitou uses it with the focus of someone who genuinely wants the patient to recover. That is the disturbing part. The same character who removes a person's mind for the King's amusement will spend days keeping someone else alive, and sees no tension between the two jobs.
Fans argued for years about whether Pitou is male or female. Togashi has not settled it, translations disagree, and the Japanese avoids the question entirely, so the fandom has largely accepted that the answer is not coming. What is settled is the effect: Pitou is the reason the Chimera Ant arc feels less like a tournament and more like a horror story with a body count.
Spelling PITOU
PITOU keeps the U that most romanisations throw away. The katakana holds a long O, and English usually flattens that, the way Toukyou becomes Tokyo. Here the official spelling writes it out, so the row ends OU rather than O. The full name is Neferpitou, but five letters is what the game asks for: PITOU.
